Little bit wider than shoulder width. I've never gone really wide stance on deads or squats.

I always lock out. The only time I don't go back down to the ground is if I'm doing stiff leg deads, and those I do in the rack on the lowest setting.

Not while lifting. My knees are in much better shape now that I don't have to run 3x a week.
